We accept most major private pay insurances. We do not accept Medicaid or CHIPS as a source of insurance. Please know and verify your benefits, including whether our pediatricians are participating providers on your plan. Please be aware of your deductibles, copays, and coinsurance. Likewise, some services may not be covered by your plan, including some services that are often routine such as vision and hearing. If a service is performed by our office, and your insurance does not cover it, then you will be responsible for the fee.
The plans listed below are generally accurate; but at times some plans may be added or removed. We recommend that you check with your insurance prior to scheduling an appointment to verify that our doctors are providers on your plan.
At the current time, we do not accept Blue Cross Blue Advantage HMO, or any Military Plans or Workers Comp Insurance.
Parents/Guardians of patients are responsible for deductibles, co-pays, non-covered services, coinsurance and items considered “not medically necessary” by your insurance company. Co-payments and anticipated coinsurance amounts will be collected at the time of check-in and will be expected prior to services being rendered. Due to the rise in deductible plans, we will be asking for an initial payment at the time of service. Beginning February 1, 2018 if you have a deductible, and have not met it, we will charge $75 at the time of your appointment. Once your insurance makes adjustments, we will send a statement with your remaining balance. If you do not have insurance, full payment will be due at the appointment. If you are unable to pay at the time of service, please inform us before your appointment. We will make every effort to work with you at that time. If a parent/guardian is unaware of their deductible or coinsurance amount, we will bill the insurance company as a courtesy. Any remaining balance should be taken care of within one (1) months’ notice from the insurance company.
Even if you have insurance, if the payment is not made by your insurance company, or we are unable to collect a fee, you will still be responsible for the fee incurred. If you or your insurance carrier makes payment exceeding your balance, reimbursement will be remitted. If payment cannot be made at each visit, or your family has an outstanding balance, you must notify our office to make arrangements in advance of each office visit.
We offer services to self pay patients on a discounted basis. Parents/Guardians of patients are responsible for making payment for care at each patient visit. If payment cannot be made at each visit, you must notify the billing department to make arrangements in advance of each office visit.

You must present your card at each visit per your insurance company and you must notify us promptly of any change in you or your child’s insurance status.
